Plan of Care State Requirements
Review Plan of Care
Within the client’s Plan of Care tab > POC versions, select the File link for specific version you wish to review.
A new tab will open with the Plan of Care and you will be able to use the lefthand menu to navigate between the different sections.
Print Plan of Care
When printing a Plan of Care, you will choose a PDF template and then pick the sections that you need to include within the Plan of Care option:
- Print Plan of Care – generate a PDF of the client’s PoC
- Print Plan of Care and Tasks – generate a PDF of the client’s PoC and tasks assigned
- Print Daily Activities – generate a PDF of the client’s assigned daily activities
- Print Plan of Care with Client Story – generate a PDF of the client’s PoC and client story
- Print Plan of Care with Client Story & Tasks - generate a PDF of the client’s PoC including client story and tasks assigned
- Print Daily MAR – generate a PDF of client’s Daily Medication Record
- Print Monthly MAR – generate a PDF of client’s Monthly Medication Record
The Print Daily Activities, Print Daily MAR, and Print Monthly MAR will not include the Plan of Care section(s) below.
Click continue which will process your request and a new tab will open with the generated PDF of the client’s Plan of Care that captures the template you selected and the specified section(s).
Plan of Care Acknowledgement
To see which caregivers have reviewed a client’s Plan of Care, navigate to the POC versions tab and under the Acknowledgement(s) there will be a View button.
As each caregiver answers the ‘Plan of Care Acknowledgement’ on the Care+ KanTime mobile app, this screen will capture those views.
Plan of Care State Requirements
For the Admin user of the branch, they will be able to go to the Care Plan tab > Interview POC Settings and configure the Plan of Care documentation that meet your office and state requirements.
You can include and customize State Requirement fields including details for Types of Services, Plan of Supervision, and Charges for Services Rendered. These fields are limited to 500 characters.
At the bottom of this section, the Plan of Care Title is where you can enter the state specific naming convention such as ‘Service Plan’ or ‘Individual Service Plan’.
The last portion is the Interview POC Print settings and is where you can select the name of each screen included in the printed Interview and the printer Plan of Care.
